Details of where the fixed and mobile traffic speed cameras are located in the County of Wiltshire England. Contains background information such as enforcement site maps and facts on the number of deaths and injuries. In Wiltshire, 3,293 people were killed or injured in 2003. That is the number of people you can get into 63 coaches! Just imagine, 63 coaches involved in crashes!! Please drive carefully! |
4. Notes for Wiltshire FA Referees.

The Information that Referees will need to enable
forms to be completed is shown below:
1) The Wiltshire County FA Ltd form may be used when reporting to Wiltshire
County FA Ltd or any other County FA in the Country (England) - please use the
drop-down facility (in the 'Word' document version) when completing the form (to
get the correct County).
2) * The FA Form must only be used when reporting to the FA for "Full" Member Clubs - this includes all Conference, Southern League & Ryman League Clubs, but not Western League Clubs, unless they are "Full" Members of the FA. The form should also be used for any Youth Teams directly connected to the "Full" Member Club. If you're not sure give the Wiltshire County FA Ltd Hon. Discipline Secretary a call.
3) The FA Code box has been omitted from input in the Wiltshire FA Form - this is deliberate, as this field is not required. The FA Code is only required when reporting directly to the FA London (see above *).
4) When entering Club details, please ensure you also include which Team? - e.g. example 1st, Reserves, Youth, U18, U16 team etc.
5) You should only use the multi-caution form for Cautions in 11-a-side matches - however if you happen to Caution a player and later dismiss him for a second Cautionable offence then you should use the current misconduct report forms to report both the original caution and the dismissal - in these cases please do not enter the details of either the original or second caution (dismissal) on the new forms!!. In other words, players receiving two cautions in a game must be reported on a standard misconduct reporting form.
6) You should send the forms to the relevant County FA or The Football Association (whichever is applicable) - Where you caution players from teams from different Counties in the same game (e.g. one team is affiliated to Wiltshire and the other team is affiliated to Gloucestershire), a copy should be sent to both County FA's involved (with all of the cautions on them) - if you don't have an address please contact your FA County Referees' Appointments Secretary for details - some County addresses may be found in the County Handbook
7) A list of NPD Numbers was emailed to all
Referees in Wiltshire by the Referees' Appointments Secretary on 16 Aug 04. An
NPD number is not your FAMOA number, but a number assigned to Referees by their
County FA. The NPD (National Participants Database) number is required for
completion of the Misconduct Report Form. An NPD number is unique to an
individual Referee and is held on the new County Administration System (CAS).
The NPD number should appear alongside a Referee's details in their County FA
handbooks.
Note from Mark Ives: County Business Development Manager, The Football Association:
If you are sending the form to a County Association please use your NPD number which is the County Registration number and is different than the FAMOA number. The FA regulations have been amended, and approved by FA Council, to permit the reporting of misconduct on these forms at all levels of the game.
If your report is for the Football Association please use the FAMOA number.
8) All other misconduct, sending offs, misconduct by Club Officials etc should still be reported using the old style forms.
9) Please read the notes on the new multi-caution forms, they are self-explanatory
10) If you have any problems please give contact your County Discipline Secretary (for Wiltshire this is Mark Edmonds, Hon Discipline Secretary), or contact your County Referees' Appointment Secretary.
11) When reporting players from Wiltshire County FA Affiliated Clubs, the new
multi-caution forms can be sent via email to Wiltshire County FA Ltd -
please send to edmondsm.wiltsfa@btinternet.com
- you will receive an acknowledgement of receipt email in return - if you do not
receive an acknowledgement within a couple of days please contact Mark Edmonds,
Hon Discipline Secretary, Wiltshire County FA Ltd on 01980 842153.
Note: Down Ampney, who play in the Wiltshire County League, Junior Division are
affiliated to Gloucestershire County FA.
